Kriel Care Specialists provides functionally driven, therapist-aligned home-based and assisted living care for patients transitioning from acute hospitals and subacute units into the home environment.

We work alongside occupational therapists, physiotherapists, speech therapists, and social workers to ensure that therapy goals don’t stop at discharge — they’re carried through safely and consistently at home.

How We’re Different from Traditional Home Nursing Agencies

Kriel Care is not task-based nursing care. We operate from a rehabilitation and functional outcomes perspective.

Caregivers are placed based on diagnosis, functional level, and therapy goals
Care plans are aligned with OT, Physio, and Speech programmes
Caregivers are trained to support — not replace — therapy interventions
Strong focus on safety, carryover, and preventing regression or readmission
Hands-on clinical oversight and active communication with the MDT
