Comprehensive Initial Assessment

Ilanie Kriel, Kriel Care’s Director and social worker, conducts an initial assessment, meeting the patient, following up with the current therapy team for recommendations on home care, and gathering background information from the patient and family on the home environment and care plan expectations.

Care Partner Identification and Orientation

Our staff supervisor and team leader identify an appropriate care partner, who then receives orientation from Ilanie on the patient’s needs and family expectations.

Caregiver Training and Facility Orientation

The identified caregiver attends training and orientation at the facility where the patient is receiving treatment, ensuring a seamless transition into the home environment.

Care Implementation in the Home Environment

Ilanie Kriel conducts a care implementation session with the patient, caregiver, and family in the home environment, ensuring that the care plan is effectively integrated and all parties are comfortable and informed.
